Community Engagement Coordinator - Essential Functions

The Community Engagement Coordinator serves as a visible ambassador for Grace Medical Home, building community partnerships, coordinating outreach resources and events, and sharing Grace’s story to connect uninsured individuals to compassionate, whole-person care. This role strengthens relationships with community organizations, volunteers, and partners to expand access to care and elevate awareness of Grace’s mission and services.

  • Serve as a liaison representing Grace Medical Home by promoting its mission, services and opportunities for involvement throughout the community and building strong relationships with community organizations, churches, schools, nonprofits, civic groups and local agencies.
  • Collaborate with partners to increase awareness of Grace’s services and strengthen referral and patient enrollment pathways, while distributing outreach materials and inviting community members to Discover Grace tours.
  • Plan, coordinate, promote, and manage logistics for community outreach events—including health fairs, church mission fairs, enrollment events, and partnership activities—overseeing schedules, materials, staffing/volunteers, vendors, and on-site support.
  • Represent Grace at community events and track outreach activities, participation, and outcomes, assist with follow-up communications and reporting.
  • Support volunteer recruitment, retention, stewardship, and recognition efforts, including coordinating logistics for Annual Volunteer Appreciation Week initiatives.
  • Assist with referral specialist stewardship and recruitment in collaboration with the Community Engagement Director and clinical leadership, including managing sovereign immunity contracts, onboarding and offboarding providers, and maintaining accurate records in Volgistics, Raisers Edge and other clinical databases.
  • Lead provider recruitment committee meetings with clinical leadership and volunteer physicians to identify specialty care needs and pursue resources to improve patient access.
  • Support organizational events, campaigns, and presentations with creative content and messaging through engaging digital content, e-newsletters, and multimedia storytelling that inspires community members, volunteers, donors, and partners to connect with and support the mission.
  • Stay informed about best practices in nonprofit communications and community engagement to continually strengthen Grace’s connection with the community.

Other Related Functions

  • Demonstrates awareness of and compliance with medical/legal issues, patient rights, and HIPAA, and complies with regulatory and accrediting agencies.
  • Practices efficient use of supplies. Maintains a clean, safe, and organized unit. Performs all other duties as assigned.
Qualifications
  • Bachelor’s degree in Communications, Marketing, Public Relations, Journalism, Community Health, or a related field (or equivalent experience).
  • Minimum of 2 years of experience in community outreach, partnership development, event planning, communications, or nonprofit engagement (nonprofit experience preferred).
  • Demonstrated ability to build and sustain relationships with community partners, volunteers, and stakeholders.
  • Experience planning, coordinating, tracking, and evaluating outreach events and initiatives.
  • Strong writing, storytelling, and verbal communication skills.
  • Experience with graphic design tools (Canva, Adobe Creative Suite) preferred.
  • Experience with social media management, email marketing tools (e.g., Raisers Edge, Constant Contact, Mailchimp), basic web editing (Squarespace preferred), and video production is a plus.
  • Highly organized with the ability to manage multiple projects, partnerships, and deadlines.
